Transcranial Doppler ultrasound (TCD) is playing an increasingly important role in multimodal neuromonitoring in pediatric intensive care. Noninvasive, radiation-free, reproducible, and readily performed at the bedside, it enables a dynamic assessment of cerebral hemodynamics. Beyond its well-established historical indica- tions-screening for stroke risk in sickle cell disease and surveillance for vasospasm-TCD is now being applied across a broad range of acute neurocritical conditions. This narrative review summarizes the essential physical and technical principles required for reliable acquisition, provides practical interpretation landmarks that ac- count for key confounding factors, and proposes bedside decision-making algorithms to support the rational integration of TCD into daily neuroprotective management.Downloads
